Referral Claims Process

  1. Make Appointment with Insurer Approved Dentist
  2. Print Out Claim Form and Take to Dentist
  3. Ask Dentist to Complete the Relevant Sections
  4. Pay for the Treatment
  5. Remember to Get a Receipt!
  6. Post or Submit Claim Form Online, Enclosing Receipt
