Eligibility

You are eligible for benefits on the day of hire if you are a regular, full-time employee (unless otherwise specified)

Enrolling Dependents

You may also enroll eligible dependents for benefit coverage. When covering dependents, you must select the same plans for your dependents as you select for yourself.

Dependents Include:

  • Your legal spouse or qualified domestic partner
  • Your children, which may include natural, adopted, or stepchildren
  • Your qualified domestic partner’s children

Your parents and siblings are not eligible dependents.

Attentive reserves the right to request verifying documentation to ensure all dependents enrolled are eligible.

Domestic Partner Coverage

If you enroll a Domestic Partner or your Domestic Partner’s children in the plan, ADP Comprehensive Services will send you a Domestic Partner Affidavit for completion. Please send the completed form to benefits@attentive.com.

Do not list your spouse as a Domestic Partner as Domestic Partner coverage has tax implications associated with it.
