Attentive offers you a wide range of medical plan options to best fit your family’s needs.
Regardless of which plan you choose, you will receive preventive care covered at 100%, the opportunity to enroll in a tax-advantage account (FSAs/HSA), and quality of care!
Attentive offers 4 nationwide medical and prescription plan offerings through Cigna Healthcare:
- One EPO plan that offers in-network coverage only.
- Two PPO plans (Base and Buy Up) that offer both in- and out-of-network coverage.
- One HDHP, or high deductible health plan, that can be paired with a Health Savings Account (HSA)
- You do not need a referral to see a specialist!
- All plans have the same network of providers, but the deductibles and costs per service will differ.
- Save money by staying in-network.
Find an In-Network Provider
Need to check if your providers are in the network or want to find a new in-network provider?
- Go to cigna.com
- Select “Find a Doctor” at the top
- Under “How are You Covered?”, select “Employer or School”
- In Search Location, enter your zip or city/state
- Choose a search category:
Select “Open Access Plus, OA Plus, Choice Fund OA Plus”
ID Cards
Your Cigna ID card for yourself and dependents will be accessible on my.cigna.com. You can even add your digital ID card to your Apple Wallet.
If you’d like a physical ID card(s), you can request one on the Cigna Portal.
Prescription Drugs

- Information about the prior authorization and decision, including decision letters, reasons for denials, and next steps to take if a prior authorization was denied
